The numbers, state by state
We are collecting every state’s public Medicaid record — the manuals, bulletins, contracts, and rules any member of the public can reach — and checking it against the federal regulation. This page is the count, for every state, with how each number was gotten and the date it was counted. A state we have not reached yet says so in plain words.
No estimate stands in for a count on this page. The one estimate we publish stands at the top, labeled as an estimate, and it never appears inside a state’s row.
Where the count stands today
The program works one state a week, in a set order, and each state’s numbers are counted in its week. Counting has begun in three jurisdictions. Their totals so far:

How big is the whole job? Projecting from the four jurisdictions counted so far, the whole public Medicaid record across all 51 jurisdictions plausibly runs 80,000–150,000 documents. That range is an estimate — projected from the four jurisdictions counted so far (roughly 500–3,700 documents per state at the North Carolina / Tennessee / South Carolina range, with California counted as the outlier it is). No state's row on this board uses it.
How to read the table
What this gauge counts: the share of the state’s public Medicaid documents we have collected and checked against the federal regulation, out of the state’s whole public set. It is not a score for the state. Until it reaches 100%, an item not yet covered means we have not finished looking, not that the state is missing something. At 100%, whatever remains uncovered is a true gap in the public record.
Whole set — the state’s whole public set: every document in the state’s State File Inventory, the list we count against. Found — documents located in the public record: the ones on file, plus the ones found and not yet filed. On file — documents we actually hold. Checked — documents collected and checked against the federal regulation. Covered — checked as a share of the whole set: the gauge above. Every counted row prints its basis — what the whole set is, and what the rest of the state’s documents are doing — on the line under the row. A dash means that step has not happened yet for that state.
In review — the state’s record is in the program’s hands now, being collected and checked, and its row says so: how much stands checked, how much remains, and when the review is projected to complete. Projected completion — a projected date, not a promise. Where the program has measured the state’s remaining work, the date comes from that measurement; where it has not, the date is the schedule target from the program’s sequence — the end of the state’s week — and the row says which.
